- Control status (inadequately controlled, poorly controlled, out of control).
- Episodes of hypo- or hyper-glycemia
- Insulin use (including insulin pump)
- Manifestations/Complications
- Circulatory complications (e.g., peripheral angiopathy)
- Hyperosmolarity, with or without coma
- Ketoacidosis, with or without coma
- Kidney complications
- Neurologic complications: specify type of neurologic complication
- Diabetic mononeuropathy
- Diabetic polyneuropathy
- Diabetic autonomic neuropathy
- Diabetic amyotrophy
- Other neurological complication
- Ophthalmic complications: specify nonproliferative diabetic retinopathy as:
- Mild, moderate or severe
- With or without macula edema
- Oral complications
- Skin complications (e.g., foot ulcer)
- Arthropathy
